Re : Ms Cassandra Kinser ,aged 32


I am writing to discharge Ms Cassandra Kinser who requires further medical evaluation. She has been diagnosed with urinary tract infection.


Today, Ms Kinser has been admitted following an assault by her boyfriend. Upon admission, she had lip laceration, abdominal pain and burning sensation while urination, lasted for several days. She was prescribed Cipro,500 mg orally twice a day for 06 days ; Clindamycin,300 mg orally 04 times for 10 days ; Oxycodone, orally one for every 04 hours and Peridox mouth wash,10 ml after food. Please note, she is sexually active and follows unprotected sexual practices; therefore, he is suspected to have sexually transmitted diseases.  


It would be appreciated if you could treat Ms Kinser for urinary tract infection as her urine appears infected. Of note, she has a review at sexual health clinic also her antibiotics need to be monitored.
