Re: Mr Joe Black

DOB : 03 June 1994


I am writing to refer Mr Joe Black who is recuperating from a motorbike accident. He requires further assessment and rehabilitative care from you service. He is due for discharge on 21 July 2021.


Mr Black met with an accident and was diagnosed with multiple fractures to the limb, spinal injury and permanent paraplegia. He was transferred to our facility following a total hip replacement and surgical spinal intervention. He has been initiated with rehabilitation and physiotherapy for recovering muscle strength, to cop-up with limited and to avoid muscle hypotrophy.


Mr Black lives with his parents in a two story house which is located on a steep hill with 10 steps at front entrance.


In view of the above details, I would be glad if you could evaluate Mr Black for further rehabilitation and to assist him regaining as much self-reliant as possible with ADLs. Please ensure that he has necessary aids to support with his mobility and ADLs. Kindly make modifications suitable for the wheel chair. Please educate and support him to adapt new modification by regular visits for 2 weeks.
