Re : Mr Alexander Jones
DOB : 01 June 1960
I am writing to request home visits for Mr Alexander Jones who is recovering from the right partial rotator cuff tear. He requires continuous monitoring and assistance from your facility. He is being discharged today.
Mr Jones presented to our facility on 30 June 2021 following a fall. On assessment, he experienced pain and weakness in the right shoulder especially when lifting arm overhead. He was performed an x-ray and MRI which revealed the right partial rotator cuff tear. He was discussed surgery ; however, he preferred non-surgical treatments. He was treated with Ibuprofen orally four times a day, cortisone injections and daily physiotherapy.
Mr Jones lives alone. He is a widower and has no children. He has type 2 diabetes mellitus and is on metformin 500 mg mane.
It would be greatly appreciated, if you could monitor Mr Jones’ blood glucose level fourth hourly which may be elevated due to cortisone injections. He needs assistance in showering and house work. Please note, he has an orthopedic review on 01 August 2011.
