Re : Ms Olivia Hawthorne

I am writing to refer Ms Olivia Hawthorne who is suspected to have intra uterine device shifted or fallen out. She requires further investigation and an ultrasound at your facility.
Ms Hawthorne was diagnosed with menorrhagia and dysmenorrhea in 2010 and was treated with cerazette. After two years, the tablet was ceased to conceive her first child. On the next year, she was prescribed combined pill cilest for worsening of persistent symptoms. The medicine was stopped for her next pregnancy on the subsequent year. On 10/10/2017, intra uterine device mirena coil was inserted owing to menorrhagia returns with menstrual cycle, for which she was on canestan.
On 02/10/2021, Ms Hawthorne presented with menstrual spotting over the last three months. She had menorrhagia, dysmenorrhea along with increased hairiness as well as greasiness of the skin. She was advised for intra uterine device removal. Today, she has undergone the device removal, which was unsuccessful because of unable to locate the strings.
It would be appreciated if you could assess and manage Ms Hawthorne’s condition. Please note, her pregnancy test was negative, and she has two children.
