Re : Mrs Jane La Paglia, aged 71


I am writing to transfer Mrs Jane La Paglia who requires follow-up care and management following her discharge today. She was diagnosed with mild dementia, pneumonia and renal failure secondary to dehydration.


Mrs Paglia was admitted to hospital on 04 March 2011 due to renal issues. She was on coma and was ventilated for a week in the Intensive Care Unit. During hospitalization, she was performed dialysis and chest physiotherapy for thrice and twice a week respectively. Currently, she has hospital acquired pneumonia and is on 02 liters of oxygen via nasal prongs.


Mrs Paglia lives with her 80 year old husband Mr Joe, who is not coping with his own care needs. They have poor nutrition and hygienic practices.


It would be appreciated if you could take care of Mrs Paglia .Kindly encourage her to maintain adequate hydration and to perform self -care activities independently. Of note, she needs urea, creatinine and blood tests. Her husband will stay in the hostel next door upto 28 April 2011.
