DOB: 24 April 1936
I am writing to refer Mr George Gale who is recovering from urinary tract infection (UTI) and requires temporary residential care following his discharge today.
Mr Gale was admitted to the hospital on 10 May 2011 with confusion, disorientation and fever following a fall. He also reported an episode of vomiting palpitations and dysuria two weeks before this incident. His urine analysis confirmed the UTI, which was treated with amoxicillin along with his regular medications, such as paracetamol, 500mg 02 tablets 04 times for osteoarthritis and Felodipine 5mg for hypertension. Currently, his vital parameters are normal. He also requires regular monitoring for non-specific colitis.
Mr Gale is a widower, who lives alone in his flat. He smokes 10 cigarettes daily and consumes alcohol socially.
In view of the above, please encourage Mr Gale to perform physical activities for at least 30 minutes due to significantly lowered mobility. Although he is keen to return home as soon as possible, he still has episodes of confusion, for which assessment for independent living has been recommended.
