Dear Mr Joice,

Re: Mr Pablo Picasso

Age: 69 years

I am writing to refer Mr Pablo Picasso who is recovering from coronary artery bypass graft (CABG). He requires continuing rehabilitative care and monitoring following his discharge today.

Mr Picasso was admitted to our facility on 09 June 2022 and underwent CABG. Post operatively, intravenous antibiotics, analgesics and antithrombotic agents were administered. Apart from that, elevation of leg for 24 hours and elastic stockings were recommended. His chest tube was removed and wound was with in normal limit; however, pain was noticed over chest and shoulder, which was managed with Ibuprofen. Daily deep breathing exercises and gradual increase in walking 04-05 minutes have been suggested, which need to be continued.

Mr Picasso has hypertension, high cholesterol and arthritis, for which he is on Perindopril 01, Atorvastatin 01 and paracetamol, 01gram four times daily respectively.

In view of the above, assessment and dressing over the wound, chest and graft site two days per week would be profoundly beneficial for Mr Picasso. It is important to avoid heavy lifting. Please note, sutures need to be removed after 02 weeks. In addition, he would require a full time -carer and meal services on his return home.

Kindly contact me for further queries.

Yours sincerely,

Charge Nurse
