I am writing to request home visits for Mr Obama who requires ongoing care and management from your facility. He underwent percutaneous translational coronary angioplasty and is being discharged today.


Mr Obama was admitted to hospital on 05 February 2020 with the complaints of chest pain and breathing difficulty along with other minor ailments. His ECG and blood investigation were revealed myocardial infarction. During hospitalization, he was instructed to avoid oily and carbohydrate rich foods and to take green leafy vegetables and fruits.


Mr Obama has had hypertension and diabetes for the past 08 and 09 years respectively and is on medication. He has the habit of smoking and drinking also follows an unhealthy diet. All medication list has been enclosed with this letter for your kind reference.


In view of the above, kindly take care of Mr Obama and provide appropriate care, monitor his medication compliance, and give instruction regarding dietary control. Monitor his blood pressure and blood sugar levels as well as provide instruction regarding daily exercise. Please note, he has a follow-up on 02 March 2022, and instruct him to avoid hard exercise for the next couple of weeks.
