Age : 76 years


I am writing to update the current health status of Mrs Oprah Winfrey who has been diagnosed with left-side ischemic stroke. She requires follow-up care and is being discharged today.


Mrs Winfrey was admitted to hospital on 03 November 2021 with the definitive symptoms of aforementioned diagnosis and was treated with tissue plasminogen activator and oxygen supplementation. During hospitalization, she was commenced on medications. She has been initiated physiotherapy and language therapy for aphasia and apraxia also mediterranean type of diet has been recommended. Currently, she has made excellent progress.


It would be appreciated if you could monitor Mrs Winfrey’s medication compliance and blood pressure, which needs to be maintained less than 140/90. Please make sure to screen her for diabetes and depression. Kindly assist her to lower the cholesterol level below 70 and encourage her to participate in social activities as well as to continue the physiotherapy and language therapy. It would be highly beneficial for her to arrange an appointments with a neurologist on 15 January 2022 and dietician to reduce her BMI.
