I am writing to refer Mr Grace Yai who was diagnosed with deliberate self-poisoning and major depression. He requires consultation and further management.


Mr Yai was admitted to hospital on 01 December 2021 with reoccurrence episode of self-poisoning, severe insomnia and low mood. He developed similar DSP episode with depression six months ago following divorce with his wife due to extra marital affair. Therefore, mirtazapine dosage increased from 15 mg to 30 mg, along with frequent cognitive therapy and counselling in Ethiopian, which showed significant improvement in his condition. Currently, he has made good progress and does not have suicidal intension.


Mr Yai lives with his mother. She does not believe in psychiatric medicines due to her beliefs in traditional Ethiopian medicine and culture, for which counselling was given on the importance of anti-depressants medications in her son’s recovery.


It would be appreciated if you could manage Mr Yai. Kindly monitor him for adverse effects of anti-depressants and continue the CBT and exercise program. Please note, avoid prescribing benzodiazepams, as he is vulnerable for self-harm abuse. Additionally, assign a language translator for conveying information.
