Re : Mrs Wilson Martha

Age : 59 years

 

I am writing to refer Mrs Wilson Martha who has had hyperthyroidism for the past six months. She requires further assessment and treatment after being discharged today.

 

Mrs Martha was admitted to hospital on 07 November 2021 with the complaints of joint and abdominal pain, constipation as well as dry mouth. She also reported of slurred speech, dizziness and fatigue. On assessment, her blood Pressure was 170/110 with unremarkable other vital signs. Her lab investigations revealed decreased TSH along with increased T3 T4. Her hemoglobin was 08 and blood sugar was more than 400. Slight bleeding in temporal bone and bulging in the right upper quadrant were revealed by MRI and ultrasonography respectively. Enlarged gland in the neck was also noticed. Thyroidectomy was recommended; however, she requested for medical management.  She was treated with medications.

 

Mrs Martha has irritable bowel syndrome, hypertension, seizure disorder and right side paralysis following Stroke. Her mother had diabetes mellitus.

 

It would be appreciated if you could take over Mrs Martha. Kindly advise her to continue regular medications and to perform regular exercise. She needs to follow the diet containing fiber, calcium, restricted salt and limited diary products.
