Re: Ms Sandra Miller

I am writing to request a home visit for Ms Sandra Miller who underwent second angioplasty as a result of coronary artery disease and angina. She requires care and support following her discharge tomorrow.


Ms Miller’s post-operative period was uneventful. During hospitalization, she tolerated semi-solid diet and fluids. Even though bruising was noted at the catheter insertion site, no signs of infections were observed. Currently, she is concerned about managing her condition at home.


Ms Miller underwent angioplasty in 2012. She has coronary artery disease, hypertension, hypercholesterolemia, and coeliac disease, for which she is on medications. She lives alone and uses hearing aids. She leads a sedentary life style, and her BMI is 29.5.


It would be appreciated if you could assess Ms Miller’s wound site for any signs of infection. Kindly instruct her to follow the recommend diet and exercise. Monitor her medication compliance. Please note, an arrangement for family visits has been suggested to avoid social isolation. She needs to avoid heavy lifting for 12 weeks and requires regular visits from a doctor, physiotherapist and dietitian. All pertinent details have been enclosed with this letter.
