I am writing to refer Mr Nasser Ali who requires urgent investigation and management for possible nerve root compression and lumbar disc prolapse. He has been transferred today.


Today, Mr Ali experienced pain in back and down left leg even after taking medications. His SLR was 10 degrees to the left leg and 30 to 40 degrees to the right leg. He also has decreased toe extension, angle flexion and pinprick sensation along with no angle reflex. His random blood glucose level has increased to 14. Initially, On 15/11/21, he had similar complaints, which did not subside with Nurofen . Therefore, he was prescribed paracetomol and anti inflammatories along with bed rest and hot water application. On subsequent days, his condition deteriorated despite prescription modifications.


Mr Ali has non insulin dependent diabetes mellitus, which is controlled by diet and exercise. Last year, his spinal X-ray revealed some narrowing of L4-5 and sign of osteoarthritis in L5-s1.


In view of the above information, kindly take over Mr Ali. Please note, orthopedic registrar has been contacted for hospitalization and to perform orthopedic assessment. All pertinent information have been enclosed with this letter.
