Dear Nurse In Charge,


Re : Mr Henry Harwood

Age : 42 years


I am writing to introduce Mr Henry Harwood who requires urgent isolation and further management as he has symptoms suggestive of COVID-19.


Mr Harwood presented to our facility on 18 November 2021 with the complainants of coughing, sneezing along with dyspnea and recurrent headaches for five days. His physical examination revealed temperature (39 °C) and heart rate (75). Therefore, he was managed with paracetamol 500mg if needed, dehydrated 500 ml DNS and nebulization fourth hourly. On observation, he has been advised to use personal protective equipment especially mask and gloves as well as drinking hot water.


Mr Harwood travelled to China on 11 November 2021. He had left lung resection in 2017. He has bronchial asthma, diabetes mellitus and hypertension, which have been controlled with medication. He experiences frequent chest infection. Of note, he is allergic to penicillin.


It would be appreciated if you could quarantine Mr Harwood. Please note, his throat swab and blood reports are awaited. He family requires assistance from a social worker. All pertinent information have been enclosed with this letter.
