Re : Ms Agnes Mabel

Age : 79 years


I am writing to refer Ms Agnes Mabel for further assistance and follow-up care. She was diagnosed with cellulitis and will be discharged tomorrow.


Ms Mabel was admitted to hospital on 18 November 2021 with the definitive symptoms of cellulitis. Therefore, she has been managed with intravenous pencillin and lasix along with oral paracetamol and Panadol. Two days later, she developed allergic reaction to penicillin; therefore, penicillin was changed to intravenous ciplox. Currently, her health condition is improving significantly, and she is on oral ciplox along with other medications, which need to be continued.


Ms Mabel has had diabetes mellitus and osteoarthritis since 2015 and 2018 respectively, for which she is on medications.


It would be appreciated if you could monitor Ms Mabel for further complications and side effects. Kindly perform wound dressing regularly and report to the local general practitioner in case of any complications. Please note, she has a follow up visit at Out Patient Department after 05 days. All pertinent information has been enclosed herewith this letter.
