Re: Mrs Sandra Peterson
DOB: 03 June 1921
I am writing to refer Mrs Sandra Peterson who was diagnosed with upper respiratory tract infection. She requires on-going nursing care and monitoring following her discharged toady.
On 20 march 2012, Mrs Peterson presented to the hospital following a fall. Upon admission, She was confused and had shortness of breath. Her blood pressure was 190/90. she was on 24 medications which was encouraged by her daughter. During hospitalization, she was commenced on intravenous saline for 24 hours and Augmentin 500mg twice daily along with her on going anti hypertensive medications . Her blood investigation and radiology reports reveled normal . Currently, She is able to mobilize with a seat walker at short distance; however, wheel chair and assistance require for long distance. She is co- operative but has mild confusion.
Mrs Peterson lives with her daughter who is very supportive. She has hypertension, moderate dementia and sometimes incontinence of urine.
Based on the above information, kindly monitor Mrs Peterson's medication compliance and educate her daughter , regarding poly- pharmacy. Please note, she needs assistance to maintain her personal hygiene.
