I am writing to update the current health status of Ms Ivan Cooper who has increased anxiety and risk of asthma attack. She requires an urgent assessment and appropriate management.


Today, Ms Cooper presented to our facility with the complainant of anxiety due to final exam in 10 days. She complained of dysphagia, hot flushes, starvation, insomnia and breathlessness. She she had increased seretide dosage upto 04-05 puffs per day and was asking about taking prednisolone, as it earlier helped her to relieve asthma. On assessment, Escalated anxiety and inappropriate asthma management were noticed. She is suspected to have side effects of prozac.


On 28 January, Ms Cooper reported frustrated, sobbed and insomnia. She also claimed that prozac was not working. Therefore, she was reassured regarding the effectiveness of prozac, and stress and asthma management techniques were reviewed.


Based on the above information, your urgent evaluation and appropriate management towards Ms Cooper 's health condition would be profoundly beneficial. Please note, all pertinent information has been enclosed with this letter
