Re: Mrs Maeve Greerson
DOB: 21 June 1961
I am writing to refer Mrs Maeve Greerson who has been diagnosed with lower intestine cancer with widespread metastasis. She requires palliative care and support at your facility following her transfer today.
Mrs Greerson was admitted to hospital on 24 September 2021 following a collapse at home. On examination, she had dehydration, nausea and severe pain. She was treated with intravenous fluids, transdermal patch and light low-fiber foods. During hospitalisation, she had occasional break through pain, for which her pain medication was increased. In addition, she was weak, disoriented and had bladder incontinence along with severe oedema in the ankles and lower legs. She rejects solid foods and tolerates fluids.
Mrs Greerson is a widow. She underwent laprotomy in March 2021 followed by partial bowel resection and colostomy. She had radiation therapy for 06 weeks in April 2021 to relieve the symptoms. She was expected to survive not more than 03-04 months. Currently, her condition is deteriorating.
It would be appreciated if you could provide hospice care for Mrs Greerson.
