I am writing to discharge Ms Juanita  who has been diagnosed with schizophrenia. She requires follow-up management.


Ms Jones presented to our facility on 01/10/2021 with decompensated schizophrenia. Upon admission, she had auditory command hallucination, visual hallucinations, delusion, agitation and aggression. She was also responding to internal stimuli with thought blocking and latency. Therefore, she was commenced on risperidone. Subsequently, she ceased reporting auditory and visual hallucination and signs of thought blocking and latency. She is able to minimize delusion and focus on activity of daily living. She has made good progress.


Ms Jones has hypertension secondary to fibromuscular dysplasia and primary hypothyroidism, for which she takes Levothroxine 38mcg daily. She is unemployed and on disability allowance for schizophrenia. She has a history of poly substance abuse and is noncompliant with medications.


In view of the above, kindly assess Ms Jones for objective signs of psychosis, redirect her from delusions and ensure her medications compliance.  Please help her maintaining behavior control and provide therapy if possible. Currently, she is on risperidone, 04mg nocte and 01mg twice daily if needed for agitation or psychosis.
