Re: Ms Guana Ferrocia

DOB: 01 June 1964


I am writing to refer Ms Guana Ferrocia who has been diagnosed with grade four haemorrhoids and is scheduled for hemorrhoidectomy tomorrow. 


Ms Ferrocia presented to the hospital with the complaint of rectal bleeding with burning pain. On examination,a slightly tender prolapse was evidenced. She also reported of constipation and hard stools. Therefore, pathology was ordered, and the results are awaited. She was performed an enema and was administered intravenous fluids. Pre-procedure assessment, post- surgery management, dosage of anesthesia, level of sedation and hospital policies were explained to her. She was provided a literature on the procedure and pain. Currently, she has signed consent for the surgery, and her vitals signs are normal.


Please note, Ms Ferrocia is sensitive to penicillin and aspirin and has no sensitivity to codeine. She has type-2 diabetes mellitus and inflamed bowel syndrome. 


It would be appreciated if you could provide further care and management for Ms Ferrocia. Please note, she needs to be in nil per for six hours prior to the surgery. All pertinent information have been enclosed with this letter.
