I am writing to refer Shirley Docosta, a three-weeks-old baby, she requires follow up care and support from your facility. She is being discharged today.

Shirley was born on 09 April 2011. Her delivery was normal without any complications. Subsequently, breast feeding was initiated and her sucking reflex was good. At the time of birth, her body weight was 3008grams; however, it had increased 34 grams on discharge. This considerable increase in weight is normal, as because of the suggestive care provided by the mother. She was perfectly alright without any kind of disease and weakness. The baby and her mother is doing well at the time of discharge.

Shirley’s mother Ms Diamond Decosta lives with her husband Mr Josh Decosta. He is a taxi driver. He has one more child, who is 07 years old.

It would be greatly appreciated if you could assess Shirley and her mother’s health condition. Please monitor her mother’s medication regimen. Kindly educate Ms Decosta regarding breast feeding techniques and importance of breast feeding.
