Re : Ms Bethany Taylor
DOB : 01 January 1929
I am writing to refer Ms Bethany Taylor who has persistent chest pain. She requires immediate assessment and management at your facility.
Today, Ms Taylor did not take her evening meal as she felt nauseous. She had trouble sleeping and complained of shoulder and neck pain. Her blood pressure was 175/95. She was administered 02 Panadol and 01 Normison at 10 pm. Further review after 35 minutes, she felt distress, pale, sweaty and complained of persistent chest pain.
Recently, Ms Taylor has the complaints of indigestion and shoulder pain. She has had hypothyroidism and hypertension since 2000 and 2007 respectively. She has glaucoma and arthritis in hands. Her current medications are Karvea, 150mg 01 daily ; Oroxine, 0.1mg 01 daily in the morning ; Timoptol Eye Drops, 0.5% 1drop each eye in the morning and evening and Normison 10 mg as she also takes non prescription medications such as golden glow glucosamine tablet and one with breakfast and vitamin C complex sustained release one with breakfast.
It would be greatly appreciated, if you could take over Ms Taylor.
