Re: Ms Maria Joseph, Aged 39
I am writing to refer Ms Maria Joseph, who is suspected to have space-occupying lesions due to intracranial pathology. She requires urgent evaluation and management.
Today, during a home visit, Ms Joseph was found in a semi-conscious state with slurred speech following a fall caused by a severe headache. In addition, she had an injury to her right arm and bruises on her left leg. On examination, her blood pressure was 150/90, pulse (100), and her left leg knee flexion and extension were limited (4/5).
Ms Joseph initially visited on 10 May 2021 with complaints of a frontal headache lasting for 6 hours, associated with nausea and blurred vision. Her physical examinations were unremarkable; however, she was suspected to have a personal dilemma, for which she was advised to take rest and paracetamol 500mg, four times a day.
Yesterday, Ms Joseph revisited with worsened persistent symptoms, including five episodes of vomiting and paraesthesia. On assessment, she was found to be distressed, and her vital signs were slightly elevated. Subsequently, she was treated with single intramuscular injections of Pethidine, 100mg and Maxolon, 10mg.
In light of the above, it would be appreciated if you could provide urgent evaluation and treatment for Ms Joseph.
